Nobody like filling out forms at the doctor's office, especially if you're with an anxious patient.  You can find the forms you need by clicking here, print them and fill them out, and bring them with you when you visit.

Boarding

  • Reservations are required. During holidays openings fill up fast; thus, we recommend making your reservations early.

  • All vaccinations must be current

  • All dogs are required to be free of fleas, ticks and other parasites.

  • Food:  We encourage you to bring your dog's regular food to avoid digestive upset and provide consistency. All Creatures Veterinary Hospital will follow your feeding instructions. If you prefer, All Creatures will feed your pet quality dog food at no additional cost to you. Meals are given twice a day, morning and evening. Lunch or additional meals may be arranged by request.

  • If your dog needs medication bring it along with delivery instruction and one of our trained staff will administer it to them. 

  • Dogs boarding 5 days are required a bath at a nominal fee to you.

  • Please review the Boarder's Checklist for additional items.

  • Office Hours:  Monday - Thursday 7:30am-5:30pm                                                          Friday - 7:30am-5:00pm

  • Saturday - 9:00am - 1:00pm

  • *Special operating hours may apply during holidays.

 

BOARDING CHECK LIST

Items you provide:

  • Food portions with feeding instructions if other than two meals a day.

  • Medication and/or supplements

  • Updated shot records

  • Updates on your dog's health (allergies, injuries, etc.)

  • Quick-eat treats (biscuits, cookies, soft treats, carrots)

  • Local contact information (if leaving the area)
